Hello, Sorry for the long post:

I am not sure what would capitalize my next steps and need some advice:

PreReqs from undergrad (graduation 2008, Chemical Engineering):

Biology I
Biology I Lab
Biology II
Biology II Lab
Chemistry I
Chemistry I Lab
Chemistry II
Chemistry II Lab
Physics I
Physics I Lab
Physics II
Physics II Lab
Psychology
Statistics
GPA: 3.01

Completed PreReqs (2014-2015):
Anatomy
Physiology
Abnormal Psychology
GPA: 3.48

prereqGPA: 3.25; although this is complicated as schools vary in this department

GRE:
Verbal: 150 45% below
Quantitative: 146 25% below
Combined: 296
Analytical Writing: 3.0 15% below

Observation Hours: 100 in Outpatient, Inpatient, School, Long Term Care

LOR: Struggling with choosing an academic professor as I have not heard back from an undergraduate professor and nervous about choosing one from an online class

Questions:
Certain schools will accept classes taken more than 5-7 years ago if an upper division science course is taken. Some schools require exercise physiology which I do not have. Some schools, I meet all of their basic prereqs but there is no mention as to a time limit and what they are looking for if courses are beyond time limit. What classes should I take next to capitalize on the number of schools I can apply to? Microbiology and Biochemistry to update my sciences for the one school that suggested it or exercise physiology and another psychology course that would meet the requirement for certain schools that will not accept General Psychology.

Also did anyone use a LOR from an online source, how did you develop the relationship to ensure a quality reference?

Low GPA, low GRE, putting all my eggs in one basket with regard to taking specific classes, is it a matter of contacting each school to determine what I need

I don't know much about the specific schools you're applying to but my advice would be this:

Retake any old prerequisite courses where you could significantly improve the grade (your old prereq GPA is 3.01 so I'm sure improvement can be made somewhere in there), and if you don't see anything about time limits on the website I would call the admissions department.
I would also retake the GRE, your scores look just at or even below some of the schools I've seen.
 
Definitely retake the GRE. A lot of schools require >300 total.

I think it would be more helpful to look at the list of schools that you're interested in, or potentially interested in, and take the pre-reqs that you still need to take. Many schools do require 2 psych classes, either clinical or developmental psychology. Some also require medical terminology, or english, or exercise physiology...

A few of my schools allowed harder classes to replace the more basic classes...so my Microbiology grade was better than my general bio grade, so the school used the higher grade for the biology requirement. Try to look for schools that take the higher grade of your repeat courses, rather than schools that just average your two grades. This helped my gpa a lot.
 
Retake any of the basic pre reqs (looks like your undergrad pre reqs) that you have Cs in (before taking any of those other classes you mention). Retake the GRE for sure. Keep working on those hours to foster a good LOR.
