HELP exam issues

So I have a problem. I put 30-40 hours into exam prep, understand all the material, show others how to do it, then get bad grades on the exams. I don’t understand how this is happening. I would love some advice on why to do about this, thank you.
 
I would recommend reviewing the exams to see what mistakes you're making. From there you can decide if you were actually struggling with the concepts or making silly errors. Do you have a history of test anxiety? This could be an issue for you if it happens often. During the exam, always check your work. If you find that you're pressed for time, try doing practice problems with a time limit.
 
It’s always just little things and I understand the whole concept just half points here and there that really get me. It’s extremely frustrating.

If it's not test taking anxiety, then it's a confidence issue. Always go with your first choice answer, and do NOT second guess or overthink.

You must build up the discipline to not self sabotage yourself in this regard,

Go visit your school's education or learning center for help in this.

I’ve heard this many times, and I just keep on switching things and after I’m left wondering why I did so. Next round of exams I’m going to imprint this in my mind and go with instinct. Thank you for the reminder!
