Help me make a list: low gpa with strong upward trend, high MCAT, somewhat nontrad

So, I am 27 years old and past mistakes have put me in a bit of a situation. I went to college just out of high school, and with no idea what I wanted from life I screwed up big time for 4 semesters. I then took 5 years off to work and sort myself out, during which I became absolutely sure I want to become an M.D. So I started over with school from the beginning, and am now majoring in Biochem with minors in molecular bio and biomathematics.
Over the past 4 years my cGPA has been ~3.7, but when my past classes are accounted for my GPA is only 3.1.
Other relevant info:
-Overrepresented (white male)
-First generation college student
-Colorado Resident
-MCAT: 36 (12PS,13VR,11BS)
-250 hours clinical volunteering (hospital ER), 400 non-clinical (homeless mission and summer camps for children with cancer)
-50 hours shadowing (primarily anesthesia and surgery)
-10,000+ hours non-medical work experience since high school
-10 hours/week rock climbing hobby (not exactly relevant, but may show lasting dedication/motivation)

I am not terribly interested in DO school, and would rather do post-bacc or grad school in order to reapply to MD if this cycle doesn't work out, so I need honest input from people about which MD schools I should apply to, and why.

Nobody will make a list for you from scratch. SDN will edit the one you bring though.
 
Sorry I should have put this up sooner. My list is essentially gleaned from other "reinvention" threads, but obviously circumstances can be different for each of those, so please let me know of any additions/removals that you all would do, and why.

I know you are all very involved in the admissions process, but it's so weird to read some of the top tier schools being suggested. No offense to the OP, it's just odd to see Columbia and Duke being suggested to someone with a 3.1. I assume those are schools that traditionally value MCAT or look closer at trends than others?

OP, purely on anecdotal evidence, I know someone who got into Cincy with a low gpa (can't remember exactly but a little higher than yours without a trend, 3.3 maybe) and a 31 MCAT.
Columbia and Duke are traditionally schools that value reinvention.
