This is what I plan to have done by the time I apply my senior year. I'm focusing on an upward trend but these things are ongoing/more intense during the summer(s).

Clinical Volunteering:
4oo hours

Hospital Volunteering:
400 hours

Research:
One intense summer in a lab, no publications.
2 academic years + 1 summer in a lab with a popular PI.
Volunteering in clinical research (time commitment unknown yet).
1 year research project in learning methods + presentation.

Shadowing:
A few doctors have said yes, so some hours this summer and more next summer/year.

Other/MISC:
Worked in a grocery store the summer before college.
Was in a program for selected Biology majors the summer before college (research/medical oriented).
Ran a 1 credit course which was supplemental to certain STEM classes.
Random medical/science clubs (no leadership ).
Medical trips to 1 country so far, more in the future.

Gap Year:
Not sure, honestly whatever I get paid to do. Might be a research assistant that looks to hire gap year students from my university or a medical assistant in a hospital. Might take more classes to boost GPA.

During years, research and clinical/hospital volunteering. Need to get grades up.

Looks good?

Everything looks good. But I would recommend focusing on your grades and MCAT right now. While extracurriculars are important, they are something you can easily improve during your gap year. GPA and MCAT however are permanent.
