hi guys,

I am INternational medical student. i had done 1 yr peds residency in us. due to family circumstances ,i got attracted to PM&R residency. i want to APPLY TO THIS FIELD I haVE AVERAGE USMLE SCORE AND good LOR from my residency program. i want to know what are my chances.

Have you done any rehab rotations?

Also you're going to need an internship as well. Pediatrics doesn't count for PM&R.

peds DOES count. IM counts, FM counts, surgery counts. as long as you have 6 months of "medicine" rotations. it doesn't specify that it must be "adult" medicine. we had a resident that came from peds.

From the ACGME, under Duration and Scope of Education:

"Physicians seeking specialization in this field [PM&R] must complete four years of graduate medical education, three years of which must be physical medicine and rehabilitation training. Of these three years no more than six months can be elective. No more than one month of this elective time may be taken in a non-ACGME accredited program, unless prior approval is given by the Review Committee.

One year of the four years' training will develop fundamental clinical skills. This year of training in fundamental clinical skills must consist of an accredited transitional year program or include at least six months in accredited training in family medicine, internal medicine, emergency medicine, obstetrics-gynecology, pediatrics, or surgery, or any combination of these patient care experiences. The remaining months of this year may include any combination of accredited specialties or subspecialties."

Interesting.

I'm not sure I agree with the wisdom of that though, because in your PGY-2 year you're responsible for (perhaps without any seniors above you) managing the acute problems overnight of rehab. patients. I would imagine these issues are similar to the typical issues of the geriatric heavy Internal Medicine inpatients you might see in the Preliminary Medicine or Transitional Year program. But they would differ a lot from overnight issues in Pediatrics.

Preliminary Surgery I would put in the "just barely" category. Some folks I've talked to who switched over from surgery said they really struggled with basic, easy issues on PM&R call.

People from OB/GYN shouldn't even be allowed in PM&R.

Ouch!! You got a point though. BUT, LSU is a categorical program that mixes the medicine and PMR months together the first two years. This means that some PGY1 interns are doing inpatient rehab their first two months. It's difficult...and you feel like a bloody idiot for not knowing what to do with the most simple calls from the nurses, but that's the same thing with the first few months of medicine wards months in a prelim/transitional program (and with much sicker, unstable patients).

so whereas prelim surgery, OB/GYN, and peds seem like they don't prepare you much for the medicine calls you get during your inpatient wards months, I don't worry much b/c they'll catch on quickly with what to do.
