Help w/ Repeat in AACOMAS

How do you list them (repeat courses)
For example: (2 classes)
General Chemistry I C (first attempt) repeat 01??
General Chemistry I A (second attempt) Last repeat 01?

Physics I D (first attempt) repeat 01? or 02?
Physics I B+ (second attempt) Last repeat_____


Hope you guys could understand what I am trying to say.

And, with classes that have W's do I list them as "0" hours like repeat or Not?
