Hello im a pre-med. sorry to ask this question, I know what's up and how it is suppost to go down, I read a Medical School Admissions book and stuff, but please just help if you can, it would be very much appreciated, thank you. Im going to be shadowing a Physician for a week or so, hopefully around 15 hours or maybe 20, but how do you ask a Physician for a LOR after only that short of a time you know?

You don't need an LoR from a physician.

Sorry, so should I just say thank you for letting me shadow you or do I need to get evidence that I shadowed her so I can put it on my App.

You should probably shadow her for more than 20 hours before it becomes meaningful experience, and you run the risk of an interviewer questioning you about an experience that may be overstated. That said, if you've done it, there's no verification necessary to put it on your application.
