Have you exhausted all your resources?

Try sending out resumes or dropping by any clinics near where you live to see if they are interested in hiring a summer assistant. Definitiely check out you local emergency clinic too because often Summer is a very busy season plus many people like to travel and an extra employee can relieve schedules.

At the same time, earning money is a good thing! If you can get a solid job working for family, it might be a wise move. You can dry shaking it up a bit by trying to find different clinics/practices to shadow - perhaps focusing on an area where your app might be lacking (you have lot of SA, find a LA and vice versa).

Don't give up looking already! Finding a job in this field in this current market is no easy task, but persistence pays off!
