Hole Punching Help

What are you're guy's hole punching tips. I remember seeing one thread where someone attached a word document along with a pic and an explanation. I searched or it but couldn't find it. From what i remember, he drew a grid an "X" where the holes were going to be, but I don't quite understand it yet.

PS The grid does work. I tried it, i just need to work on speed now.
