Ok, now that I am in the bowels of internship, my mind is beginning to wander into happy thoughts of rad onc residency. Ahhhh....

Anyway, I have a quick question for current/past residents...how do most programs handle the big holidays? I'm thinking Christmas/New Years. I know for ward residents, vacation is usually split (half of people get 5 days over christmas, half over new years). Rad Onc seems a bit different though...is there a standard for holidays? Does everybody just work up until the main day and then return the next?

If you can't tell, I am on call and doing anything to avoid cross cover chores...
 
actually the ward system you describe is not common to my knowledge. most dont get 5 days. you have so many vacation days and you take it as you will. there may be some places that require one of them to be over the holidays. most places will just limit the days people need to be there like a weekend. Radonc you just work up till the day and get the holiday off. depending upon when the holiday is they may (like at thanksgiving) runt he machines one day over the holiday with a call abck up team on .

you can take one, two, three, or more days around holidays. however, a resident and attending are usually on call during every holiday. so, if you happen to be on call, then you will need to stick around the area. are first years on call more often than not? usually...but it depends how big the residency is, if they care if you live far aways, or if you request it in advance...
 
Our system splits the holiday call among all of the residents. First years are typically on the hook for X-mas and Thanksgiving, with senior residents entrusted to hold down the fort on occasions such as Flag Day and Groundhog Day.

As radonc noted, the time on call goes down as one advances. I think we have a general set-up (for 8 residents) of: 1st yr- 11 weeks, 2nd year- 7 weeks, 3rd yr- 5 weeks, 4th yr- 3 weeks.

I concur. We have a similar program @ my (future) RadOnc program. The newbies are responsible for call during the "big three" weeks of Christmas, Thanksgiving, and New Year's. 12 residents: 8 wks R1, 6 wks R2, 4 wks R3, 2 wks R4.

I typically take Christmas each year. All the other residents either have kids or family in the general area, so they should have a chance to spend time with the fam. Not that it's that bad. In a year-and-a-half of residency I've had to come in for an emergency once and only been paged 3 times after 10 PM. 😍
