Hello...Just found this forum and am impress at how active and informative it is....A hospital near me managed by Cardinal has started this remote order entry during normal operating hours in addition to after hours....this eliminated 4 FT pharmacist...is anyone seeing this at their hospital or area? Is this yet another threat to our jobs? Thoughts please....and no I'm not trolling...lol

I second that. I occasionaly do remote order entry for my hospital (when someone calls in sick, it's unexpectedly busy, or I can't make it in for whatever reason) Working from home kicks @$$

My hospital also uses cardinal e-source to cover from 10 pm to 6 am. They are very very expensive and we are trying very hard to get rid of them. However, it is very difficult to find well trained night time hospital pharmacist. It is a tough position since you have to make all kinds of decision by yourself with not much of help. Our new vice president already told my DOP to try to hire pharmacist and get rid of e-source, but I don't think it will happen soon. Unless they lower the price, I don't think it will be a threat to us.

what are your thoughts on hospitals doing this during daytime normal operating hours? That is my concern.

Depends on the volume/types of service. We spend around $30,000 a month. So hiring 2.5 RPh FTE would be cheaper.

That's because Cardinal is ripping you off. You can get the same service for around $10,000 to $15,000.

Master, do you mind sharing your knowledge how to get the price down? They charge us per line items, so only ways to lower the price is d/c some services like ED orders... what else can we do??

Cardinal won't budge much. And charge per line item is insane if you're a busy hospital. Just get out of the contract and find other service. It's dime a dozen now. Better way is to pay per hour or a flat fee.
 

It's a business model. And you know exactly why they do it...charge for convenience, coupling with other services (i.e. Pyxis, management, etc.). But you'll see the same in every industry...not just healthcare. Of course, competitors will have their niche too, and rightfully so, doing something "better" or "cheaper". We'll always knock the big guy on top taking something mainstream though.

Hey guys, right now I am looking for a night hospital position, do you think I should stick w/ a larger hospital (ie 500+ beds) b/c of the likelyhood that electronic order verification might be utilized (I would assume that a smaller institution would be less likely to justify 24/7 services and would search for alternatives when budgetary difficulties arose, such as the times we are in now where institutions are strapped)

Anyway I would appreciate your opinions guys, or any response for that matter, even if it is just caverject or WVU jokin around 😉.

On a side note, it does seem that the way cardinal is presenting this is to expand our opportunities rather than diminish them, by promoting such services mainly to hospitals w/o 24/7 pharmacy services.

Goal is to have a box full of flies before I go fishing again before the browns spawn....

My hospital is actually the opposite. We are getting RID of our off site pharmacist. She covers the evening shift from 1p-11p along with out 2 in house evening shift pharmacists.

The issue is she ONLY enters orders. Any new consults (kinetics, anemia management, etc) or orders that need to be clarified get stamped by her "as needing onsite pharmacist review" and left in the queue. This sucks for our other pharmacists especially once the day shifters who are on the floors go home at 6. This means the onsite pharmacists spend a lot of time cleaning up her unfinished orders.

Anyhow, at least in our institution off site pharmacists are not all that helpful and we are getting rid of ours and getting an additional in house pharmacist.

Makes sense...it's a natural progression for those who have mastered full contact origami.

Yes, it's called the Fly Tying Triathlon. I'm not sure I'll be touching those tough ones yet. Also, I'll concentrate on small nymphs first because I tend to fish cold tail waters from damn where trout feed on small bugs.

Now if I'm in Florida...different story.

Well, that was stupid decision to use remote order entry while your pharmacy is open and pay the pharmacist a full salary. It's ok, stupid directors make stupid decisions.

It makes sense for remote order entry for a one man hospital to fill in during business hours when the pharmacist has to be out of the hospital on emergency etc...but why pay a full pharmacist salary for someone sitting at home cherry picking orders?

Nah, it's at home. You have to have some experience at most companies though. Like 3 years minimum...usually...

No idea, call and find out. If I can't find two pharmacist jobs somewhere, that's exactly what my wife is going to do...

2 jobs? id be happy with ONE job right now. 🙁

Can't just move? Hell, my hospital is hiring...

id love to, im stuck bc of family obligations. i may have to move out of state eventually to work and send money bcak here to cover the bills here and where i live (never thought id have to do this in pharmacy)

Some operations require you to work out of an office whereas some mom and pop operations do it out of home through hospital VPN. It's not as simple as verifying prescriptions rather processing hospital physician orders. There's a lot of regulatory gray with this process and I foresee state boards will soon take a serious look at this procedure.

And the usual hours are 11p -7a during weekdays and 6p-8a weekends... basically chitty hours.
