How and when do you get scholarships from specific schools?

Do you have to apply for scholarships? Or do schools just offer them to you without you having to do anything? Also, do they let you know you're offered one when you're accepted, so that you can make a decision knowing about it. Or are you informed way after you've made your decision on where you're going to school?
