How can I find the transcript matching form?

Hello Guys:
I could not find the transcript matching form on the AADSAS application for 2010. It says that it is in the colleges attended section next to the colleges' name on the left but I can't find it. If any body knows how to find it please let me know.

Hey I took a screen shot of mine for you, hope it helps. Also, make sure you add your institution first otherwise it won't be available.
