I'm starting to look over the micro stuff and realize there are more bugs than i'll ever be able to cram into my head. How detailed are the actual questions on the exam, ie. will they ask about what is encapsulated, a specific protein on the cell membrane, etc? Or do they ask more broad questions like ....pt. presents with so and so symtoms, what bacteria or virus is causing this?

In my case I felt the Pharm and Micro questions were the most direct and consequently the most detailed questions on Step 1. The answer choices seemed to be more similar as well. There were a mix of very specific questions and basic questions.

I agree with RJ: micro wasn't that bad. I had alot of pseudomonas (fruity, burn victim, CF patient, gram negative bacillus rod), I had family gets sick on sushi (vibro parahaemolyticus), had monoarticular arthritis (but gonorrhea wasn't an option).

Pharm was likewise pretty straight forward, although there was one drug I had never seen before in my life! Thankfully, it was regarding toxicity effects, and I played the sesame street's "which one of these is not like the other" game, and there were 4 parasympathetic effects and 1 sympathetic listed, so I went with sympathetic, baby!

I think you would be doing some humanitarian work by telling us the name of the drug 🙂 You can write the correct letter, followed by a random letter, back and forth untl it's complete. We can all figure it out, and you wouldn't particularly disclose the "real" question 😀

Better idea, tell us the drug this way. What drug do you see above? Here is a clue, the letters are in sequential order.

Just list 5 drugs (4 of them very common such as spironolactone, acetaminophen, lithium, and valproic acid) and tell us that they are very important to know as future physicians. It's honest.

Are you splitting (you'll know what I mean tomorrow)? Seriously, we need to know the drugs so that when our patients are on them, we won't have to go digging in our coat pockets for our Tarascon.

Lyme, people, Lyme!

Yeah I think this is true, although Lyme Dz often causes an initial polyarthritis it usually settles down in one joint--often the knee

The chance the word Lyme or Lyme's Disease to show up on Step 1 is 1%. Rather, Double BB's (although I prefer Double DD's in my women). Borrelia Burgedorf aka Bilateral Bell's

An even higher yield "Borrelia" is the one that causes recurrent fever. If you know the answer to this, I might be impressed.

USF
You give good info. Do you or anyone have a reference for Vibrio parahaemolyticus being the most common bacteria in Sushi??
I looked in several sources and I am unable to confirm this.
Thx in advance

He's right. It is in the Bug Cards. A twist to this question is if instead the microorganism caused a postive romberg test. What is the organism?

nothing to be impressed about--borrelia recurrentis and hermsii, both cause relapsing fevers and transmitted by louse

I wouldn't be alarmed. My post was a litmus test to see if anyone remembered Goljan talking about Relapsing fever. Sure enough, Hidden nailed it. It looks like I am not the only one that can spit out Goljanese at a drop of a hat.

Now only if I can get a hold of the bookstore at Oklahoma State that sells the 2005 Pathology Notes for his students. I would even pay the bookstore for FEDEX shipping. Maybe Idiopathic's questions on his 2004 exam will be included in the new notes. I know one thing, if it is included Goljan wouldn't have had to look up the answers for Idiopathic's questions like the ***** that missed hyponatriema for Legionella.

Wait, I want to add an addendum to Legionella. Hey Dr. Goljan, hypophosphatemia and an elevation in AST and ALT are also seen in Legionella! Give me props in your next Kaplan Audio!

"One of my best students told me that Legionella is also associated with elevation in transaminases. Don't forget that. Everybody in this room will get that question."
