How do I answer the "Future Career Plans" prompt?

Some of these prompts give 200 words to explain. Just how much do adcoms expect applicants to have planned for their future?

I want to serve underserved populations in urban areas, specifically those that I have already worked with as a volunteer. I want to increase preventative care by increasing awareness and outreach to populations that need it most (at risk youth, foster youth). I want to do primary care.

Im also interested in doing research, continuing in the field that I worked in as a research technician the past few years. I want to take one year to do a research fellowship to learn how to better bridge my clinical interests with my research interests.

^ Am i doing this completely wrong?
 
FWIW, I think they want to know if you have a plan and why. You seem to be on the right track explaining this, IMHO.
 
FWIW, since you mention wanting to go into a specific medical specialty, add a sentence or two saying something along the lines of, "Although I am currently interested in blankety blankety blank, I remain open to exploring other specialties throughout medical school."

Wanting to do primary care doesn't require that kind of disclaimer. Planning on neurosurgery, plastics or orthpaedics does.
