i printed the form. do i just send it like a regular letter or something? and how do i get the money order? (sorry! i'm a noob.......) and how do i calculate the tax? it only shows ones for IL and D.C. but i live in TN..

also how long would this be taking?

please help me~!!! thanks~!!!

Print, fill it out, and mail it to the address on the bottom of the paper. I think only IL and D.C. pays tax so you would leave it blank. You can choose to pay extra for shipping if you want it quicker.
