How do internists specialize?

Can someone fill me in as to how internists can pursue a specialty after completing their residency?

Finish residency, apply for a fellowship, spend a year or so training, and then become an attending in whatever field you did your fellowship in. Fellowships might be thought of as extensions to your residency that you apply for once you complete your residency.
 
Fellowships.

Example: 3 year IM residency + 3 year Cardiology fellowship = Diagnostic Cardiologist

or

3 year IM residency + 3 year Cardiology fellowship + 2 year Interventional Cardiology fellowship = Interventional Cardiologist.

Everything after residency is completed through fellowships.

Actually, based on the link I provided, it seems that you apply during your second (or possibly as early as your first) year of residency.
