How do people feel with doing blocks or practice exams?

When I do blocks of kaplan (63%) and uworld (68%) I feel like I have no clue what Im doing for the majority of the questions. Ill finish the block feeling like WTF and then end up doing relatively ok.

Do other people feel this way? I never feel like Im moving through a block very confident about answer choices. Of course these %s correspond to decent step scores, but doesnt the real exam require more like 80% correct for a good score? Am i fooling myself by thinking I may have a couple to miss when taking the real thing?
 
I feel like absolute **** when I take most practice tests and UWorld as well, and another guy mentioned he feels like he knows nothing when he takes tests too. Even though I've been doing well (around 82% average on my last 15-20) UWorld, it feels like I'm not sure about most of my answers, or I'll see a few choices that seem like they could work depending on how you interpret the question. I think it's pretty normal.
