How do prelims work??

Forgive me if this is obvious, but I'm a bit unclear as to how to apply to prelim programs. Suppose I interview at program x do they guarantee me a prelim spot or do I have to apply separately with no guarantee. Does this vary by program? If so how do I know which programs guarantee a prelim, or at least an interview?

Any input on this would really be appreciated..
