How do programs find out which state you are a resident of?

as programs generally give preference to residents of their own state, ie. cali.

but it doesn't say anywhere on the app who would they know?

With the exception of some Cali programs, nobody really cares where you're from.

The answer to your question is "they can't." They can infer a connection from the location of your med school, your undergrad institution or your current address. Otherwise, they won't know.

It matters infinitely less than you think it does.

It asks on the application what city and state you were born in.
 
I've been a resident of 4 states other than the one in which I was born.

Yeah, I agree, although it doesnt make a HUGE difference, there are schools outside cali that are also looking at ur undergrad/med school location. I know for a fact case western in ohio is giving preferential interviews right now to local residents/students who went to ohio schools, although i do think they'll give out interviews to out of state applicants (non-AOA'ers) later.
