How do schools know if it was an online class?

how do schools know if the class was an online class or a face-to-face class?

The only way they will know is if it is shown on your transcript. This is school-dependent. I took over half of my classes online (everything that didn't have a lab) and it doesn't say anything about it on my transcript.
 
Correct me if I'm wrong, as it's been a few years since I've seen AMCAS, but I believe you have to check a box in the course entry if a course is taken online. Of course, if it doesn't show up on your transcript that the course was online, there's no easy way for AMCAS or schools to know, but that's up to you if you want to lie.
