How do you get an excellent LOR?

What's a good way to ensure that you'll get an above-average LOR? I go to a large school and I feel like I don't know my professors well enough for an excellent LOR, might only be able to get an average one. If your LOR writer is a professor, do you guys go to office hours a lot to make sure they really know you? When is the best time to ask for one? Also, do you guys give your LOR writer something like a brief outline of your career goals to help them write one?
 
Yes. For large classes make sure to go to office hours or make your face known in some way. What I did was first email the professor about requesting a meeting to discuss the LOR (I had already spoken to them in person). At the meeting, I brought a folder with a mini autobiography, resume, a copy of my PS, and any significant academic work conducted in the class (usually a research paper). Usually, we'd then talk about who I am, what I do on campus, why medicine, etc. I think this works best.
