How do you start practicing dentistry?

I have a few general questions for dental students/dentists out there..so you get into dental school, you pass your boards, you graduate and residency and all that but then what?

How do you actually go about starting a practice? Do you have a year or so that you take to "get ready"? (e.g. buying a place, relocating, purchasing everything you need, finding staff, etc) or do you try to find a job as an associate somewhere first? I know not everyone opens a private practice straight out of D-school, but what is the main path taken assuming you don't want to specialize?

How long does it take to actually get patients in the door? I'd imagine it's a little bit like starting a restaurant. How do you market and get patients in? Do you start looking in D-school?

Lastly, people always talk location when choosing dental school. Is it ideal to go to d-school in a state you want to practice and live in? Or would relocation be a good idea? For example, if I wanted to live in VA would going to VCU be my best bet because patients may be familiar with the school?

Please shed some light on these questions. They're general questions, but I'm looking for specific questions. It's hard, I know
