Dear all,
Well, we all got different ways when it comes to Rx verification.
How do YOU verifiy the Rx?? Do you verify the refills like an original prescription?? Do you verifiy the name and date of birth in each prescription??
Feel free to share you unique way of verifying rx...

Verify everything everytime.....Come up with a good system and do it the same way no matter how many to fill or how busy it gets.
 
At drop off i get the name, and write the dob on all scripts.

At pickup i check the name and DOB or address.
 
do you just go through the name, dob,date, drug, strength, directions, quantity, refills, once?? or twice???
 
Ill read everything multiple times very quickly and havent had any problems. I always check refills with the same deligence as I check new ones. Ive caught many mistakes from my partner and floaters on refills.

Dont slack just because someone checked it before you.
 
get Date of birth, and allergy information on all Rx's when dropped off (so as to update allergy/conditions prospectively). Verify a pt identifying factor (address/DOB/ID) when picking up...

Verification- check bottle/ensure product is consistent and correct - match label to screen/ match label to Rx. Ensure pt information is correct, date written, refills, directions, prescriber, etc. Keep appropriate records (Ex: CII logs), and check every Rx for omissions of important details. And match everything to professional sense. Check the copay and make sure insurance issues are resolved, if it's cash, make sure the patient knows. Take a glance at their prior medication history, and identify dose changes, therapeutic duplication, etc, to counsel the patient on. And definitely counsel all new Rx's.

sometimes i close my eyes and just initial. then get pissed from the phones
 
that sounds scary...but i've seen a lotta rph's doing that.🙂
