How do you verify your shadowing experience?

I'm applying this June.
I did dental shadowing in 2005. (~40hours)
I know some schools require shadowing experiences.
But do they require you to get some kind of verification letter from your dentist?
Or do you just give them information about the dental office/dentist.
It's been a while, so I'm just afraid that my dentist would not remember me 🙁

not that i'm aware of. I guess they assume you will be able to tell vivid and convincing stories of your experiences there, rather than have mommy sign your permission slip haha
 
Some schools ( I know Loma Linda does) require you to get them to sign a form verifying your hours, but on the AADSAS application you just put some really basic info about the office in, like what city it's in... and I can't remember if you need to include a contact number or not.

I think the AADSAS just has you list the hours and possibly who you shadowed. I don't remember anything about a contact number.
 
i thought tufts requires you to do something like this?

You're right... here's what it asks for:

Name of supervisor, Position Title, Brief description of activities, Total Number of Hours, Position Type, Month/Year of Start Date, Month/Year of End Date
 
Ohio State is the only school that asked me to verify my shadowing hours. I guess that the schools assume you are telling the truth because if they find out that you lied on your app. you are pretty much screwed.
 
I know UF also requires you to have the dentist you shadowed verify your hours for your secondary, I guess it just depends on the school.
