How do you write about neglect without blaming anyone in essays?

Use more subtle language, such as "not properly taken care of"
 
" I remember a patient who was brought into the hospital after she grew unresponsive; the patient had bedsores all over her body due to her immobility. "

It is implied that she should have had someone turning her regularly but it doesn't call out a caretaker specifically.
 
What does it matter if you blame someone for something that's clearly their fault? It's not like you're calling out a specific person by name or anything, some people just don't pull their weight and I don't think there's much of a need to shy away from mentioning that
