How exactly are grades/classes presented to schools?

If you have a lot of transcripts or whatever, do the schools on pharmcas just receive like 1 final sheet of all classes attempted and the credits earned/grade earned? Or do they just send the transcripts to the schools themselves and they organize it? Anyone know?
