How hard is it to get fellowships

After residency how hard is it to obtain fellowships? Like say someone finishes FM residency and wants to do primary care sports medicine fellowship how hard is that to get? Or after orthopedics residency sports medicine fellowship or any fellowship?

I suggest that you concentrate on getting into med school first.
 
Depends heavily on the fellowship and the "parent" specialty. For example, all of the psychiatry fellowships aren't competitive, and many spots - sometimes even a majority - go unfilled each year. In contrast, some internal medicine specialties are quite competitive (e.g., cardiology, GI) and fellowships in other subspecialties can be quite competitive.
 
I think this is a wrong forum to ask, maybe you should go to med students forum?
