How hard is to get a IM fellowship? From a community program

“I am getting burnout from studying so much to get 4.0gpa and be top of the class. How hard will it be to match in Cards or Gastro if I dont go to a Top IM program. I want to have a balanced lifestyle in medschool cause I need to rest and do something else apart from studying. I know I will stop being in the top percentile, But can I still match in a decent IM program being middle of the class?“

I understand where he is coming from, cause our school foster a competitive environment.

If your friend doesn't like hard work now, he's going to like it a lot less in residency when he's in his late 20s and has no work/balance at a community program working long hours, spending free time cranking out manuscripts, networking, potentially applying for a chief year instead of just keeping his foot on the gas pedal, matching a upper tier IM residency with his potential, and coasting in residency to a fellowship.

Also, the comparison's kind of extreme. Someone who's at the top of the class doesn't go from there (projected upper tier IM applicant) to a community IM program even if they take their foot of the gas.

tl;dr: the earlier you invest, the bigger the benefit...general rule in life.
