Integrated IM-fellowship programs?

Dumb question. IM-pediatrics, IM-psych, and IM-EM residency programs all exist, so why don't IM-GI, IM-pulm/crit, or IM-Hem-Onc programs also exist?

General surgery has integrated pathways now (vascular, CT, and plastics all used to be exclusively gensurg fellowships and now they're increasingly integrated programs) so could IM do the same?
 
I think the idea is that it's very important to be a good internist first before you become a GI doc, cardiologist, etc.

And if these integrated programs do pop up it'll probably be at ivory towers first, and would be crazy competitive.
