How important are 4th year elective grades?

So far my first 3 years have been smooth sailing, 260+ Step 1 and 2, Honors for every clerkship except primary care. Lots of ECs, one very strong LOR so far from 3rd year, working on a research project with presentation in June. Planning to apply for IM and subspecialize.

I'm in 4th year right now on my first medicine specialty consult elective rotation and am having some bad luck. The fellow on the service refuses to let me see consults on my own or present them to attendings, he pretty much ignores me the entire day no matter how many times I offer to help with seeing patients, writing notes, etc. The attending is the one who grades me but at this point I haven't been able to demonstrate that I'm capable of anything, I'm pretty sure she just thinks I show and that's it (which is pretty true at this point). All medicine electives at my school are H/HP/P/LP/F and I have a feeling I'm going to be poorly graded. Can anyone comment about 4th year grades and how much of an impact they'll have for residency applications? I am also not sure if the comments from this rotation will appear in my MSPE.

It never got brought up for radiology interviews. Had a few prelims that brought up my H in ICU but thats about it. I dont think it really matters.

They are not important at all. My school doesn't even let you honor them.
 
My understanding is that elective grades (unless there are outright failures) are less important to residency programs as they often won't have a way to compare you to other students since only a handful of people will do the rotation. The core rotations, however, are where they can really see how you're doing.
 
I suppose deans letters can vary by school, but at my school the deans letter only has extended commentary from the core 3rd year clerkships- there are no quotes from evals on anything 4th year, including sub-I's. Just 4th year grades.
