How important is UG tier level? Academic trends

I am wondering whether medical schools take into consideration the tier level of your undergraduate degree. Would UCSD be considered mid or top tier?
Also, transferring from a community college, I performed significantly better here at UCSD than I did at the JC. In all honestly, I am still trying to figure how this is the case. Would this upward trend in my academics be an interesting point to explain when I apply?

Unless you went to an Ivy/MIT/Stanford, it doesn't really matter where you went. UCSD is considered a good school and won't hold you back by any means, as long you have the stats to back it up. Upward trends are kind of expected, and in your case self explanatory. It is great that you were able to do well after transferring, as it can dispel worries about your academic fortitude.
