How is the IF of first author pubs from undergrads evaluated?

So my lab has an opportunity to publish in a Frontiers Special Edition. Most people I've talked to have admitted that Frontiers is known for letting really anything in and not rejecting anything. I'm okay with that - at my stage I'll gladly take a low impact publication. But I was wondering if adcoms would see a publication from Frontiers with an asterisks? Or is it more of a situation where a better journal pub would help more, but not having one won't hurt you?

I don't know that specific Frontiers journal, but some of the Frontiers journals have decent impact factors. If it's PubMed indexed, I can't imagine how a published manuscript would hurt your application.

It is highly unlikely it will be analyzed with this level of granularity. A first author pub as an undergrad will be viewed positively.

Also OP, some Frontiers journals are great with good impact factors

The LoR from you PI will be more important than the pub, discussing how substantial your contributions were, character/scientific aspects for md/phd career, etc.
