How should I document shadowing or volunteer experience

I need to start doing both of these two things soon, but I was wondering if there is a correct way to record hours done for proof later on.

I'm sure some hospitals or clinics will have a volunteer form to fill out and probably an area to get signed off, but how do I show proof for shadowing a dentist or volunteering in a private practice?

A letter of reccomendation from them works nicely.

I would make a little list of the names, dates, and contact information for each of the places you volunteered at and keep it handy. If a school wants to verify your background and they would like to contact your references you can just give them that information.
