How significant is this downward GPA trend and how would I go about explaining this in application essays/interviews?

Freshmen: 4.0/4.0
Sophomore: 3.97/4.0
Junior: 3.75/3.633
Senior: 3.73/graduated
Cumulative: 3.85
No grades received were lower than a B.

Should I mention what led to this in any specific essays or only if asked?

That is not a downward GPA trend. That is a normal fluctuation that is very common.

Now, this may come as a shock to you, but one does not need a 4.0 GPA to get into medical school. Hell you don't even need it to get into Harvard Stanford class schools.

Also, your grades are not a measure of your value as a human being or an assessment of your self worth.
