how to ask for LOR?

I have used Interfolio, and it is effortless and easy for the LOR collection process. If you can afford the annual fee, definitely get it.
All my recommenders are familiar with Interfolio since they have been writing LORs to many students.
 
Yes, a letterhead usually includes the logo of the institution, some sort of contact (address, email, etc). For example, if you ask a professor from the English department, the letter should consist the logo of the university, the department's name and maybe its phone number.
Almost all letter writers will include an official letterhead, but it's a virtue to remind them. Especially professors and healthcare business people. They know a LOR without a letterhead isn't going to be perceived as much.

"Hi Professor ___. I am applying for medical school in ___ and was wondering, given your knowledge of me as a student and your time commitments, would you be willing to write a strong letter of recommendation for me? I have attached a copy of my personal statement, a document of things to touch upon in the letter, and my CV. If you are ok with writing the letter, I can send you interfolio link. Thanks so much!"

Once you send the interfolio link, remind them that they need a letter head, date the letter for the current year, and their electronic + hand signature.

Thanks for the information, guys. If I'm applying this cycle, when's a good date to have all my letters in by? I'm taking the MCAT May 19th.

As soon as possible to interfolio. Profs get busy and things happen, so plan ahead.
