How to ask for that last LOR?

Hi everyone,
So, I have 4 LOR's for my packet. Two from Biology profs, one from a legal studies class, and 1 from my position at the nursing home that I worked at.
1 of the bio prof's letter is in, so is the one from the legal studies class, and the one from the senior care facility.

However, my advisor just informed me that the other bio prof has not written his yet. I'm not gonna be getting my committee packet until early Aug ( to be complete by Labor day).I also emailed him a couple of weeks ago and thanked him for agreeing to write me a letter ( I was emailing him to talk about something else). He wrote back to that email and also said that he messed up his foot and was having surgery on it. I felt like maybe just leave him alone for a bit while he gets that sorted out, along with something with funding for his lab ( he's a PI).

I got the email that advisor doesn't have his letter last week.

How do I go about email-ing him again? What do I say? She ( advisor) said to just...ask him again...but like...how.

"Where's my letter?"

Or alternatively, express your understanding of his situation, explain how the letter committee is just waiting on his letter, and see when he thinks that the letter can be done. I would also start looking for a back-up letter writer as well in case this falls through. Good luck
