Medical How to disclose a conflict of interest from dating a med school faculty member?

I am filling out an application with a prompt asking about conflicts of interest. I briefly dated someone on faculty at the school in question. How should I disclose this?

Just say you are previously acquainted with Professor X and so he probably shouldn't be assigned to review your application. They won't ask specifics.

He might not pick it up to know to recuse himself

I agree with above, you should disclose but in vague terms. While not explicitly required, and though it is highly unlikely he would attempt to affect your application in either direction, it is probably best practice (and provides you maximal protection) to just remove the possibility entirely.
