How to discuss an incomplete on an application?

I was looking through my transcripts and noticed I had a class listed as previously graded as incomplete. The only problem is that I didn't request to take an incomplete in the class, so I am not sure why it shows up as one. How do I discuss this in an application if/when it is required?

First discuss it with your professor and registrar to see whether a mistake has been made.

I plan to discuss it with the registrar, but I took the class over 4 years ago. So I don't think the professor will remember anything.

Do you have a copy of an earlier version of your transcript without the erroneous INC?
 
Nope, looks like it was always listed as an incomplete, I just didn't realize it until now. Was a weird class though I took it over winter break. Even still an incomplete at my institution requires documentation hopefully the bursars office will realize that isn't present.
 
Find why it’s classified as that, try to fix it, if not then explain it by what the registrar says. The only way this will look bad is if you try to skirt the blame for it and adcoms perceive that.
 
If you can't find another explanation, consider that your professor may not have gotten grades in by the deadline and therefore everyone's grade (or at least the ones he hadn't submitted yet) were marked incomplete, and once he submitted the actual grade the transcript was updated to reflect that. If so, that's a very easy explanation for the medical schools that require one!
