How to finish the ECE for CAAPID?

It's said need to submit the ECE with the Transcript Matching Form. I'd like to know how to finishe Transcript Matching Form. Fill it by myself and then do I need to give it to my school register?

As it's said:
To assure prompt handling, complete the Transcript Matching Form. Print the form and submit it to your school’s registrar.
and :
Registrar’s seal and/or legible signature included on transcript

so what's that suppose mean? what's that transcript for? or just seal on the form? but there is no sign on the form to ask people seal there... I'm confused..

I wouldn't trust anyone with my SOP and Personal Statement. This maybe genuine or maybe a gimmick for some nice samples of SOPs and Personal statements. Who wants to take the risk? 🙂
