How to provide proof of background check?

Now I'm in the process of applying for cleveland clinic electives. Applicants are required to provide proof of background check. However, I can't find any forms of background check that can be downloaded from their website. So how do I provide proof of background check? completely confused
I have tried several times but still can't get access to the contact person of cleveland clinic.
