How to remind my professor about a LOR

I had a professor agree to write me a letter of recommendation a couple of months ago, but, he has not turned it in yet. I was wondering what would be a way to remind him without seeming impolite.

"Hi Professor McAwesome,

I just wanted to thank you again for agreeing to write me a letter of recommendation for medical school! It really means a lot to me to have your support as I start this journey, and I am very grateful for it. I just wanted to let you know that I will be submitting my application in the next couple of days, but it won't be marked complete until all of my letters of recommendation are in. I know you're extremely busy, but would you be able to give me any sort of update regarding when you think you might be able to submit it? I only ask because medical school applications are very time sensitive and the earlier my complete application is in, the better! Thank you very much and have a wonderful summer!

I hope you are well. I just received a message from the Pre-Med Committee asking me to remind my letter writers about the upcoming deadline (May 25, 2016). I just wanted to follow up with you to see if you would need any other materials from me at this time-- if so, I would be happy to provide them. Again, thank you for your help and support. I am grateful for your time and consideration.

This is the message I sent to my letter writers. Every single one of them finished within the day after I sent this. Hope this helps.

I'm still waiting for two of my professors and I asked them in February. 🙁
 
"Hi Dr. Doe, I hope you're doing well. I just wanted to thank you again for being willing to write a recommendation letter for Med school for me. I'll be submitting my application soon and it won't be complete until all letters are in. Is there anything else you need from me to finish your letter?"
 
All of the above or send them a little token and "thank you" not in the mail! That usually seals the deal.
 
A friend of mine sent a handwritten thank you note and small box of chocolates to thank them for agreeing to write a letter for her current medical school application. Worked like a charm.
