If I need an individual letter writer to send a letter to AMCAS, do I just need to give her the letter request form? Assuming she is signed up with AMCAS.

Yeah give them the request form and have them mail it to AMCAS.

Or you can have them upload it to Interfolio and forward it yourself to AMCAS.
