Just type on a piece of paper that you want your score sent to:

Arizona School of Dentistry and Oral Health
Admissions Office
800 W. Jefferson
Kirksville, MO 63501

Incude the appropriate fee for the score to be sent & mail it in!

Arizona is a division of the Kirksville DO school -- which is located in Kirksville, MO. I was informed that the application screening/processing will always take place in MO, since Kirksville Osteopathic School is the school that has the say as to who enters the Arizona SOD (think of it this way: Arizona SOD is merely a satellite campus for the Kirksville DO school).

If you send your stuff to Arizona (such as the physical address of the school), it may get lost or never get forwarded to MO.

In response to a recently deleted post:
s mutans,

I don't know when you called them, but they are well aware of the School of Dentistry, and it is their biggest admissions project right now (they are more concerned with it than their entering DO class, since the DO school is well-established). Reportedly (we know how that goes!) Arizona should be sending out secondaries at the end of this week/beginning of next week.
