I'll also add that Medical Economics published a several-part series on how to do it. I found it helpful. Someone also wrote a book on it which I also found helpful. Both can be found by digging around google.

Looking at opening up my own practice in the next few months, already begun credentialing, and attorney/business establishment. Still want to read/listen/learn more.
