I can't wait to shorten the gap from "decision to initiate therapy" to "delivery of therapy" from the hours-to-days it takes during medical internship to the seconds-to-minutes it'll take in the OR/ICU. Ahhhh.

It's been a bit.......interesting around here.

I'm posting this after the O.B.A.M.A. (One Big Ass Mistake America) health bill.

I'm worried.

A little.

Heres what I know:

1) I'm fourteen (uhhhh...thats one-four) years into this gig as a private practice anesthesiologist and I still spontaneously awaken before my alarm sounds at the RIDICULOUSLY early time of 0530. SO....obviously I love what I do. Truly. I love the trade. I enjoy the operating room environment. I enjoy the fact that I can interact with patients as long as I want...and if I reach the point where I'm not enjoying it anymore, I just slipp'em a mickey in the IV.:laugh: I enjoy bantering with surgeons. I enjoy working my ass off in an effort to make the operating room efficient.

2) I enjoy the fact that if you've got a good "rep" as a hard working consultant physician, the physicians you are consulting to don't f u ck with you. And if they do, you just write them off as being a total dick. Those types are few and far between where I work.

3) Unless you are a primary care doc who is at the top of the consulting hierarchy, ALL SPECIALISTS CONSULT TO SOMEONE. Heart surgeons to cardiologists. Cardiologists to the primary care docs. Where do you think orthopedists get their total joint patients? PRIMARY CARE DOCS. GET OVER IT. GET OVER THE FACT THAT WE CATER TO SURGEONS. SO WHAT. Does that bother you THAT MUCH? REALLY? If it REALLY does, what else bothers you? Do you wanna take a surgeon to the fair and go to the bell tower with a sledge hammer and see if you can hit the bell before he does? Wanna drop your scrubs and measure each other's schlongs? Where does it stop? WHO CARES.

4) If 3) above bothered or insulted you, consider primary care.

5) Physicians as a whole may take a hit with this new obama bill...BUTCHA KNOW WHAT? KNOW WHATS REALLY BEAUTIFUL ABOUT IT? (please refer to 6)

6)Insurance companies are like the mafia. Insurance companies are LEGALIZED EXTORTION COMPANIES. This health bill is gonna PUT A NINE MILLIMETER TO THE DOME concerning insurance companies. They cant drop you if you get a long standing illness. They have to take you if you have a long standing illness. There is NO ceiling on how much your policy will pay for a long standing illness.

7)I hate insurance companies. Whether you are a patient or a doctor, when you look at the hippocracy (sic) they BREATHE... dont wanna pay for patient care....dont wanna pay doctors who provide patient care...MEANWHILE THE F UKKING CEO IS CRANKING IN TWENTY MIL A YEAR.....gimme a break.

8) When it comes to putting the insurance companies in their place, I like what One Big Ass Mistake America did.

9)I think anesthesiologists will always make a great living.

10) I really like my job.

Interestingly, I've reflected quite a bit about catering to surgeons. I like Jet's attitude. Who the f.ck cares?? We all answer to someone, and in PP, like Jet states, it's the PCP's that really DO have the "power".

My gf works in an IM office as a PA. General IM, outpatient. They get ALL KINDS of little goodies from the various specialists during the holidays etc. The consultants treat them REALLY well, because no matter how in-demand cardiologists may be in some areas, they know where their bread is buttered.

Any astute medical student can even see a degree of this "bow down" when watching a surgeon interact with an internist. They need/want the referals.

So, anesthesiologists have surgeons as "internal customers". Who cares???

Hooked up with Noyac tho at Dragos which was sweet.

Always good to hook up with a friend that is in the same profession and has been in a main stream snowboarder video.

The literary project is harder than I imagined. I'm hoping to make it a reality soon.

As an aside, my literary coach is among us... pretty famous dude in the literary world...he's applying to med school soon...dude makes full time money on the side:laugh:...I realize how hard it is to write stuff that people wanna buy...having a friend that has done it and is still doing it as a "side job" is very humbling.

Kinda like having Bill Gates as a homie I guess.:D

I consider Brasco to be, perhaps, one of the most perfectly written screenplays in a long time. One could learn MUCH story from that movie.

I don't know, I have a feeling that they are laughing all the way to the bank. They supported this legislation rather strongly as it increased the subscriber base by how many million? On who's dime?

They are given a defined profit margin so it doesn't matter how many big claims they pay out, they will just increase premiums to match this pre-defined profit margin. They now have a captive audience of mandatory customers so they don't have to worry too much about folks dropping insurance when the rates go up. (although the penalty for not having insurance may be to small to effectively force everyone into coverage). All in all it is a pretty good deal for them.

I suspect that the ultimate goal of this legislation is to expose them for the extortionists that they are so that they die a festering, bloated death when physicians and patients finally become fed up with them.

It is nice to know that there are some bigger sticks to get them to pay up, but I am not thinking that claims processing is going to get any more streamlined any time soon.

Good thing we love the job.
