it's mainly all supportive- monitor for fluid/electrolyte disturbances, hypertension, AKI, neurologic dysfunction, anemia/thrombocytopenia- may need to give RBC or platelet transfusions, etc.
if you initially suspect EHEC, you actually aren't supposed to give antibiotics or anti-motility agents as it can increase risk of developing HUS
 
Symptomatic mainly ( controlling the BP etc ) but nowadays plasmapheresis is being used in many centers
