As someone looking to go into cards, what type of candidate would be viewed as more competitive for fellowship?

Candidate A: from second tier IM program like Duke or UW and is one of the best residents in his/her program.

Candidate B: from top IM program like BWH or MGH, but is viewed more as middle of the pack or above average in his/her program.

This is assuming all other things are equal such as research, and LORS.

From my experience .its candidate B even if he is below average unfortunately its all pedigree

All other things are never equal.
If you are in the northeast they will take the average Harvard person.
If you're in the South or Southeast, Duke is not considered a "2nd tier" residency and they wouldn't necessarily prefer the Harvard person IMHO. By the way I don't really like Duke or anything, but I'm just pointing out that there tends to be regionalism in these types of selections and also people base how they rank candidates on "who you know" and who wrote your LOR's, etc.
