I really am hoping for some direction.

I am an MS3 at a top 50 school in the Midwest. I am curious about the chances of an applicant like me getting in? This is who I am on paper thus far.

URM
USMLE I : mid 240's
Preclinical grades: average
Clinical: so far HP's (med, peds, OB)
Research: undergrad & during med (no publications) topics non Rad-Onc
I am quite involved in school (organizations, sports, leadership).

Any advice on how to make myself more competitive would also be great.

what's URM?
its possible. Do very well in clinicals and get great letters, try to get some research and read the faq.
 
I would say consider doing a year-off research fellowship and do RadOnc research. does your school have a rad onc program?

Go to your department and get a project started. Even if you don't get something published, you might get a spot, so I wouldn't recommend taking a year, unless you decide to not do any research at all. On paper, you're definitely accomplished but lacking research might hold you back. Not that it matters, but I think there might be an advantage in being an URM. Not like there is very many in rad-onc, nor is there many applying, so it's hard to say if it does help out.
