I have almost consolidated all of the things necessary for the pharmacy tech license, and I have a few problems with the application:

1). What kind of envelope should I send in? Would a 9.5" X 12.5" Photo/Document Mailer be ok?

2). I have not opened my high school transcript; should I open it before sending it in?

3). Should I really tape, and I mean use regular-ol' tape to just...tape my picture on the application? That sounds a bit strange, and I would have preferred to staple it on.

4). Do I need to send the checklist in along with my application?

5). Should I include a cover page inside of the envelope? Should I say anything to the board to let them know that I am applying for a pharmacy tech license?

6). should I include the Affidavit of completed coursework or Graduation for pharmacy technician sheet if my qualifying method was by using a certified Pharmacy Technician Certification?

7). and this is perhaps my greatest of question:

Should I send my application to the address listed on the top left corner of the application?

I think this is the adress:
1625 N. Market Blvd, N219, Sacramento, CA 95834

I appreciate any help you can give on these questions; hopefully I can start looking for mroe relevant works after my license had been given to me.

I dont know what state your in but CA literally had step by step instructions. I had no help I did it myself. Don't put anything in the envelope that they don't ask for.
1. The checklist should be in the envelope
2. If it says tape then tape, if it says glue then glue. If it says attach then figure out the best way to attach
3. DO NOT OPEN YOUR HIGH SCHOOL TRANSCRIPT, it will make it invalid. However if it says get a certified copy then you must open it and go to a place that does certified copies.
4. No cover letter is needed. Simply have all pages needed and don't leave anything blank that needs to be filled out.
Also send it where it says to send it. There is a instruction page that tells you exactly where it should be sent

Everything sounds good, except I am now confused about the the certified transcript part, since I have heard another thread saying that an official copy of the transcript is a certified transcript:

Also, should I include the Affidavit of completed coursework or Graduation for pharmacy technician sheet if my qualifying method was by using a certified Pharmacy Technician Certification?
